I hereby grant permission for JourneyDance, (referred to as "JD") and/or any person(s) or entities authorized on JD's behalf to Record me during the JD Level One Teacher Training anytime from May 25, 2007, through October 14, 2007. Such Recordings may consist of photographs, audio, video, or any combinations thereof. I understand that these Recordings may be broadcast live across the Internet, as well as stored for future broadcast "on demand" across the Internet.

I grant permission for JourneyDance and/or any person(s) or entities authorized on JD's behalf to use these Recordings for the following applications:

demo reels
promotional videos, DVDs, and CD-ROMs
display on the World Wide Web
public relations and press materials
advertisements
product demonstrations at trade shows
sales demonstrations given by JourneyDance and/or any person(s) or entities authorized on JD's behalf
training materials
television or radio broadcast

I grant permission for JD and any other person(s) or entities authorized by JD to make copies in any form or media of the Recording made of me and described above, including digitized copies or written transcriptions. I further grant permission for JD and/or any person(s) or entities authorized on JD's behalf (i) to edit or alter the Recording (referred to as an "Altered Recording"), (ii) to distribute, broadcast, publish and/or transmit all or any portion of the Recording or an Altered Recording via any media including computerized means (iii) to incorporate all or any portion of the recording or an Altered Recording in derivative works or compilations in any form or media and (iv) to use my name, likeness and biographical material in connection with any of the foregoing, all to the extent and in such fashion as they deem necessary or desirable.

I understand that I will receive no compensation for any use of the Recording or Altered Recording.

I hereby release JD, related entities, and their agents, personnel, trustees, directors, officers and employees from any claims or liability regarding any use that may be made of the Recording or Altered Recording in accordance with this Consent and Release.

I certify that I am 18 years or older and have read this release and understand all its terms. I execute it voluntarily and with full knowledge of its significance.

Brief text:
If JD is forced to postpone or cancel for any reason, JD will refund all deposits proportionally.
Deposits can also be credited toward a future JD Teacher Training.

If you need to cancel, and you let us know more than 3 weeks prior to the start date; JD will retain US$500 as a credit toward a future Teacher Training and refund any remainder.

If you need to cancel and you let us know 3 weeks or fewer than 3 weeks prior to the start date; JD will retain 50% of the total tuition as a credit toward a future Teacher Training and refund any remainder.
